Referral instructions:

• All patients (whether accessing clinical services, research studies, or both) require a referral form (links: PDF | Fillable PDF) completed by their family physician, nurse practitioner, or psychiatrist.

• Under "Reason for referral", please include "ALMANAC", a brief description of the main issue, and specify whether this is a general clinical referral or for a specific research study.

• Under Medical Services Requested, please check the box "Mood/Anxiety".

Eligibility criteria:

• Adults 60 years of age or older;

• Confirmed or suspected mood disorder (e.g., major depressive disorder, bipolar disorder) or anxiety disorder;

• Able to access a ground-level office, with or without assistance;

• Independent with basic activities of daily living (ADLs).
